How to Treat a Sore Throat Without a GP: The Pharmacy First Approach

Most sore throats will be resolved on their own within a week. But when the discomfort is significant and you need support quickly, your local pharmacy can often help without the wait for a GP appointment. Through the NHS Pharmacy First scheme, trained pharmacists can now assess, advise on and, where clinically appropriate, supply certain medicines for sore throats.
What Is the NHS Pharmacy First Service for Sore Throats?
Pharmacy First is an NHS-funded service that allows community pharmacists to assess and manage seven common conditions, including sore throat, without the patient needing to see a GP first. To understand how the full scheme works, read our guide: what is the NHS Pharmacy First scheme and how does it work.
At Ham Pharmacy Group, our pharmacists are trained to carry out a structured clinical assessment using the Fever PAIN or Centor scoring tool, which helps identify whether a bacterial infection may be involved.
What Sore Throat Medicines Are Available Over the Counter?
For the majority of sore throats, which are viral in origin, self-care and over-the-counter remedies are the most appropriate approach. These may include:
- Paracetamol or ibuprofen to manage pain and reduce temperature
- Medicated lozenges or sprays containing local anaesthetics or antiseptics
- Adequate hydration and rest
Your pharmacist can advise on the most suitable options based on your symptoms, age and any existing medicines you take.
Can a Pharmacist Prescribe Antibiotics for a Sore Throat?
Under the NHS Pharmacy First service, pharmacists can supply antibiotics for sore throat where the clinical assessment indicates they may be needed. This is not automatic. Antibiotics are only appropriate when there is a reasonable clinical basis to suspect a bacterial cause.
Most sore throats do not require antibiotics, and responsible prescribing helps to protect antibiotic effectiveness for everyone.
Who Is Eligible for Pharmacy First Sore Throat Treatment?
The NHS Pharmacy First sore throat service is available for patients aged five years and over. You can find full details and book a consultation via our Pharmacy First sore throat service page. If your symptoms suggest a more complex condition, your pharmacist will advise on the most appropriate next step.
Ham Pharmacy Group serves patients across Kingston, Richmond, Surbiton, Teddington and Twickenham.
When Should a Sore Throat Be Seen by a GP?
Whilst most sore throats are self-limiting, there are circumstances where you should seek further medical attention:
- Severe difficulty swallowing or breathing
- A high fever that does not respond to over-the-counter medicines
- A significantly swollen neck or drooling
- Symptoms lasting more than three to four weeks
- Recurrent episodes throughout the year
In any of the above situations, your pharmacist will refer you appropriately.
Frequently Asked Questions
Yes, under the NHS Pharmacy First scheme, trained pharmacists can supply antibiotics for sore throat where the clinical assessment justifies it. This is assessed using a recognised clinical scoring tool and is not automatic. Most sore throats do not require antibiotics.
Your pharmacist will ask about your symptoms, how long you have had them, and carry out a brief clinical assessment. Based on this, they will recommend self-care, provide over-the-counter medicine, or supply a prescription-only medicine where clinically appropriate. The process is typically brief and confidential.
If your sore throat is accompanied by difficulty breathing, severe swelling, a very high fever, or symptoms persisting for more than three to four weeks, seek GP advice. Your pharmacist will guide you if a referral is needed.
